Medical and General Permission Slips |
In order to participate in any activity, offsite or at TPC, a completed Medical Form must be filled out and given to Jim Isphording. Also, in order to participate in some offsite activities, a permission slip for that activity must be filled out and handed in. This and the other forms can be found on the Home Page or obtain one from Jim Isphording. |
